What to expect of Medication Assessment and Deprescribing
A free ten minute consultation to discuss your needs, answer your questions and book an appointment to meet with you and or your famiily in-person or virtually.
In our first appointment I interview you to collect information about your health, medications, concerns, goals and your other health providers.
As part of completing the assessment I may contact your other health providers for missing information if necessary.
When my assessment report is ready to review with you or your family I book a follow-up appointment.
In the follow up appointment you receive a copy of the assessment report.
We review it together, prioritize suggestions and recommendations and decide what is the best way to proceed.Some clients prefer that I consult with their physician and others prefer to consult with their physician themselves.
Cost
- Ten minute phone consultation: no cost.
- Thirty minute initial in-person or virtual meeting.
- Completion of the assessment and
- Thirty minute follow-up in person or virtual meeting.
- Total: $220.00.
- Subsequent 30 minute appointments or service: $50.00.
